Providence Swedish Chehalis Family Medicine will offer free sports physicals to Lewis County middle school and high school students who need the required physical exam to participate in school-sanctioned athletics during the upcoming academic year.
The no-cost exams will be offered for the third consecutive year at Providence Swedish Chehalis Family Medicine, located at 931 S. Market Blvd., Chehalis, from 4:30 to 6:30 p.m. on Wednesday, Aug. 19.
To participate, students must provide completed sports physicals forms with parental signatures. Forms can be obtained at the clinic or by contacting Providence Swedish Chehalis Family in advance of the event at 360-767-6300.
Students under 16 years old must have a parent on site during the exam. Students who are 16 years old or older can attend without a parent but must have a parental signature on their forms. These sports physicals do not replace recommended “well-child” exams conducted by a family medicine provider.
